Coalition

Jefferson County Communities That Care (Jeffco CTC) is a coalition of over 120 members, including youth-serving organizations, government agencies, community members, parents/caregivers, and youth, who work together to improve youth well-being across the county. Grounded in prevention science, Jeffco CTC works collaboratively to strengthen protective factors to keep youth on a healthy path and reduce risk factors that can lead to adverse health and well-being outcomes. Jeffco CTC’s equity-focused, data-driven priorities include: preventing youth substance use, supporting mental health and wellness, and increasing safety and connection. The coalition’s work is guided by youth voice and addresses disparities impacting youth of color and LGBTQ+ youth, as well as those furthest from opportunity.

History

Jeffco CTC was established in 2016, although its history dates back to 2012 as the Jeffco Connections Prevention Subcommittee.  Originally based in Jefferson County Public Health, Jeffco CTC became a project of the Colorado Nonprofit Development Center (CNDC) in 2023.

Strategic Plan

Jeffco CTC is currently making progress on the coalition’s 2024 – 2028 Strategic Plan, which outlines coalition goals and strategies. The 2024 – 2028 Strategic Plan is available to review by year in the following formats:
